As artificial intelligence becomes a pivotal battleground in health tech, Epic Systems is leveraging its market leadership to integrate advanced AI solutions, putting pressure on startups and incumbents alike.
Epic Systems, the leading electronic health record (EHR) provider, is at the forefront of integrating artificial intelligence (AI) into healthcare. With 43.7% of the acute-care EHR market and installations in over 3,700 hospitals, Epic has a significant advantage as it expands its AI capabilities. The company's aggressive entry into the AI space is reinforcing its market position and creating challenges for both startups and established competitors.
According to a recent survey of 112 senior health system leaders, including CEOs and CIOs, 71% describe their organizations as "Epic-first," with 80% expecting this bias to intensify over the next three to five years. This sentiment underscores Epic's growing influence in the healthcare IT landscape.
Epic has been proactive in developing and deploying AI solutions across various aspects of healthcare. The company introduced AI Charting, a feature that ambiently listens during patient visits and drafts clinician notes, streamlining documentation processes. At its 2025 Users Group Meeting, Epic unveiled several AI tools: Art for Clinicians, an AI scribe; Penny, a generative AI copilot for revenue cycle management; and Emmie, an AI solution for patients. The company revealed Curiosity, a medical foundation model, and MyChart Central, a consumer hub.
The uptake of these AI solutions has been significant. As of February, 85% of Epic's customers were live with generative AI across Art, Emmie, and Penny. This widespread adoption highlights the trust health systems place in Epic's technology and its ability to deliver practical, user-friendly AI tools.

The AI boom presents both opportunities and risks for investors in the healthcare IT sector. For startups, the challenge is clear: competing with a market leader like Epic that has deep pockets, extensive reach, and a loyal customer base. According to the survey, 64% of health system leaders are likely or very likely to purchase from net-new startups, but this number rises only modestly to 76% for established incumbents. This indicates that while there is still room for innovation, the window for gaining significant market share is narrowing.
For investors in Epic, the company's AI expansion offers a compelling growth story. The integration of advanced AI solutions into its EHR platform not only enhances the value proposition for existing customers but also positions Epic to capture new business. However, investors should be mindful of regulatory risks and the potential for increased competition as other tech giants and startups continue to develop their own AI capabilities.
The healthcare IT market is dynamic, and the rapid adoption of AI technologies will likely reshape the competitive landscape. As Epic continues to innovate and integrate AI into its offerings, it solidifies its position as a leader in the industry, making it an attractive investment for those looking to capitalize on the AI-driven transformation of healthcare.

He believes most market participants still underestimate AI's long-run deflationary effect on knowledge work.
