With CDC vaccine guidance in limbo for nearly five months, four major medical societies released evidence-based recommendations for this fall's respiratory virus season, aiming to give doctors and families the clarity federal health agencies have failed to provide.
Think about what it's like to sit in a pediatrician's office this fall, trying to decide whether your child needs a flu shot, a COVID booster, or both. Now imagine your doctor isn't entirely sure what the official guidance says anymore, because the usual source of that guidance has gone quiet. That's the situation facing millions of American families as flu and COVID season approaches, and it's why four of the country's leading medical organizations decided they couldn't wait any longer for Washington.
On Wednesday, the American Academy of Family Physicians, the American Academy of Pediatrics, the American College of Obstetricians and Gynecologists, and the Infectious Diseases Society of America released joint recommendations on COVID-19, RSV, and influenza vaccines for the 2026-27 season. The guidance leans on an independent scientific review conducted by the Vaccine Integrity Project, working alongside the American Medical Association. It's essentially a stopgap, built by the medical community itself, to fill a hole left by a federal advisory process that has been in disarray for the better part of a year.
The Vaccine Integrity Project's review is not a casual glance at the data. Researchers combed through hundreds of peer-reviewed studies published since August 2025, adding to work they did during the 2025-26 season on vaccine effectiveness, safety, and how these diseases spread through communities. The conclusion echoes what public health officials have said for years: these vaccines still meaningfully reduce the odds of severe illness, hospitalization, and death, and the newest evidence turned up no fresh safety concerns. JAMA published the full evidence reviews for RSV, influenza, and SARS-CoV-2 on Wednesday, and the project built an interactive tool so clinicians, researchers, and curious members of the public can search the underlying studies by virus or by population group.
"Immunization continues to be one of our best tools for preventing severe illness from respiratory viruses," said Michael T. Osterholm, the Vaccine Integrity Project's executive director, in a statement. "This year's review confirms that the newest evidence remains consistent with research showing these immunizations reduce the risk of hospitalization and death among the people most likely to experience serious disease."
To understand why medical societies felt compelled to act, you have to go back to last year. For more than six decades, the CDC's Advisory Committee on Immunization Practices, known as ACIP, served as the country's trusted referee on vaccine science. Career scientists and outside experts reviewed the data in public meetings, then issued recommendations that doctors, insurers, and parents could rely on as a common baseline. It wasn't a flawless system, but it was a transparent one.
That system fractured last year when the Department of Health and Human Services dismissed ACIP's sitting members and replaced them. In January, the reconstituted committee and the CDC issued a sweeping revision to the childhood immunization schedule, downgrading six major vaccines from "routinely recommended" to a category called shared clinical decision-making. In practical terms, that meant those vaccines would only be recommended for higher-risk children or after an individual conversation with a doctor, rather than as a standard part of childhood care.
Medical groups pushed back hard. The AAP and other organizations sued HHS to reverse the changes, and in March a federal judge in Massachusetts blocked significant portions of the vaccine overhaul, ruling that officials had acted unlawfully. Yet the administration kept moving in the same direction. In late May, the White House issued a directive backing HHS's decision to cut the number of required childhood vaccinations and instructed federal agencies to fall in line. Last month, President Trump signed an executive order calling for a further reduction in required pediatric vaccinations, reinforcing the revised schedule HHS had already pushed.
The net effect: CDC immunization recommendations have been effectively frozen for nearly five months, even as the underlying legal and scientific disputes remain unresolved.

Bruce Gelin, a member of the Vaccine Integrity Project's Board of Advisors, put it bluntly during a press briefing this week. "Unfortunately, today the federal government is not fulfilling that responsibility in the way that it historically has," he said, referring to the CDC's traditional role. He noted that ACIP, at its best, brought together experts and career scientists to systematically examine safety, effectiveness, and disease patterns, and that its public deliberations gave everyone a shared source of evidence-based guidance.
That shared source is now missing, and the viruses haven't taken a break to wait for it. "Clinicians will still have patients to care for. Parents will still have decisions to make for their children," Gelin said Tuesday. "And older Americans, pregnant women and people with underlying health conditions will still need to know how best to protect themselves. We cannot allow the breakdown of the federal process to create a vacuum in evidence-based vaccine guidance."
AMA President-elect Sandra Adamson Fryhofer echoed that urgency. She spent years as the AMA's liaison to ACIP and watched its review process up close. "For decades, physicians like me have relied on federal vaccine recommendations because they were based on science and careful review," she told reporters Tuesday. "With the massive changes at the federal level, trust is waning. Changes to the childhood schedule by the administration over the last year were made without any new scientific evidence, and that's created confusion for both physicians and our patients."
To help fill the gap, the AMA surveyed physicians and ran patient focus groups over the summer. What they found offers a useful blueprint for how trust gets rebuilt: doctors want concise materials like one-page clinical summaries, and patients judge vaccine information by three things: whether it's consistent across sources, who's behind it, and whether it feels authentic rather than scripted. Patients also want balanced information that lays out both risks and benefits, coming from credentialed experts rather than press releases.
Acting on that feedback, the AMA launched SpreadTheFacts.org, a digital hub containing immunization materials for both clinicians and the public, covering COVID-19, RSV, and flu, the three leading causes of serious respiratory illness each winter.
Vaccine guidance isn't just bureaucratic paperwork. It's the difference between a grandmother getting her RSV shot before winter hits and going without because nobody told her clearly that she should. It's the difference between a pregnant woman making an informed choice about a flu shot and stumbling through conflicting headlines. When the federal referee walks off the field, someone has to make the call, and right now that someone is a coalition of medical societies leaning on independent science rather than political appointees.
Gelin framed the stakes simply: people should judge vaccine advice by asking whether the evidence was high-quality and systematically reviewed, whether qualified experts were involved, and whether the process was transparent enough for anyone to check the work themselves. "Whatever happens to the federal vaccine policy process," he said, "we cannot lower that scientific standard." For families trying to protect their kids and aging parents this winter, that standard, wherever it comes from, is what will actually keep people out of the hospital.
