A leading urologist argues that the minutes after a metastatic prostate cancer diagnosis matter as much as the medicine itself, and that real-world data, not just clinical trials, should guide those conversations.
Getting told you have cancer changes the room you're standing in. For men diagnosed with metastatic castration-sensitive prostate cancer, or mCSPC, that moment often arrives without warning. Many patients feel fine. No pain, no obvious symptoms, nothing that would suggest their body is fighting something serious. Yet the diagnosis demands immediate, life-altering decisions.
Dr. Benjamin Lowentritt, who directs prostate cancer services at United Urology Group, has spent years sitting across from patients in exactly this moment. His conclusion is straightforward: how a doctor handles those first conversations can shape whether a patient stays engaged with treatment for years to come.
Prostate cancer is common. Roughly 1 in 8 men in the United States will be diagnosed in their lifetime, making it the most frequently diagnosed cancer among American men, excluding skin cancers. Treatment has improved substantially over the past two decades. Even so, recurrence remains a real threat. Up to half of patients see their disease return within ten years of initial treatment, and that recurrence carries meaningful risk of progression. mCSPC itself accounts for about 15% of new prostate cancer cases diagnosed annually in the US.
Here's the tricky part. Doctors typically catch prostate cancer through a PSA blood test, a routine screening tool. But confirming that the cancer has spread, becoming metastatic, requires additional imaging: PET scans, CT scans, MRIs, sometimes a bone scan to map exactly where the disease has traveled. That process takes time. And in Lowentritt's experience, that waiting period, uncomfortable as it is, becomes the window where trust either forms or doesn't.
Patients need space to absorb the diagnosis. They also need to understand what role they'll play in the months and years ahead. That can't happen through a doctor talking at a patient. It requires a back-and-forth.
There's a common misunderstanding about what shared clinical decision-making means. Some physicians hear the phrase and assume it means stepping back, letting patients make calls that might not serve them well medically. Lowentritt pushes back on that framing directly. Shared decision-making isn't about surrendering clinical judgment. It's about building a two-way conversation where a doctor's expertise and a patient's lived reality both carry weight.
Small details end up mattering enormously. Take pill size. A patient who struggles to swallow large tablets is a patient at risk of skipping doses, and skipped doses over three, four, five years of treatment add up to real consequences. Yet clinicians don't always ask about swallowing difficulty, dosing frequency preferences, whether a medication needs to be taken with food, or whether a spouse or caregiver is actually the one managing the pill bottle at home.
This is where treatment flexibility becomes clinically relevant, not just a convenience. ERLEADA (apalutamide), an androgen receptor inhibitor approved for both mCSPC and non-metastatic castration-resistant prostate cancer, is the first and only next-generation androgen receptor inhibitor available as a once-daily, single-tablet oral treatment. It can be dispersed in noncarbonated water, orange juice, or applesauce, or given through a feeding tube for patients who need that option. None of that flexibility matters if a doctor never learns a patient needs it. Medication adherence is one of the strongest predictors of long-term survival in this disease. Asking the right questions early isn't a courtesy. It's clinical strategy.
Trust also depends on the evidence doctors bring into the room, and here Lowentritt raises a point that deserves more attention than it usually gets. Randomized clinical trials remain the gold standard for proving a drug works. But trial populations don't always look like the patients sitting in exam rooms across the country. African American men and men of Caribbean African ancestry face a higher risk of developing prostate cancer than other groups, yet they've historically been underrepresented in the clinical trials that generate approval data.
Real-world evidence helps fill that gap. It doesn't replace randomized trials, but it complements them by showing how a medication actually performs across the full diversity of patients using it in everyday practice, not just the narrower group enrolled in a controlled study. For Lowentritt, this isn't an abstract policy concern. It shows up in his exam room regularly. When Black patients raise concerns about being underrepresented in trial data, he can point to real-world evidence showing strong outcomes in populations that actually reflect the US patient base. That reassurance, he says, translates into patients who stick with their treatment plan longer.
Equity in cancer care isn't only about who gets access to a drug. It's also about whether the evidence base behind that drug speaks convincingly to the person taking it.
There's a version of medicine where the doctor decides and the patient complies. Lowentritt is describing something different: a partnership built the moment a diagnosis is delivered, one where clinical expertise and personal circumstance both shape the plan. He puts it simply. He wants to be treated as a partner in his own care, not a body receiving instructions, and he assumes his patients want the same thing.
That framing matters more now than it did a decade ago, because mCSPC treatment options have genuinely expanded. Patients today often face real choices between different treatment pathways, not just one prescribed route. Shared decision-making gives them a way to actually own that choice rather than simply accept it.
None of this diminishes the seriousness of the medicine involved. ERLEADA carries significant warnings, including risks of cerebrovascular and cardiovascular events, fractures, falls, seizures, severe skin reactions, and lung inflammation, all of which require careful monitoring and honest conversation between doctor and patient. That honesty is precisely the point. Shared decision-making doesn't mean softening the risks. It means making sure patients understand them well enough to make an informed choice, and feel supported enough to follow through on it. That's what keeps people engaged in treatment they'll likely need for years, not months, and engagement, in the end, is what saves lives.
